Choctaw Nation Eye Clinics are staffed with licensed optometrists and fully trained optometry technicians.

Eligibility

  • Must have a medical chart with a CNHSA health facility or:
    • CDIB, letter of descendancy, or tribal membership card from a federally recognized tribe
    • Social security card
    • Photo identification card (driver's license)
    • Current insurance information such as Medicare, Medicaid, or Private Insurance
  • For children under 19 years of age, the following documents are required:
    • Child's CDIB or a copy of either parent's CDIB
    • State issued birth certificate or hospital birth record issued by an Indian Health Service or tribal facility

Additional Information

All Eye Clinics have a supply of eyeglass frames available. Patients may select and purchase frames along with the lenses. Unfortunately, services for contact lenses are not currently available. In addition to eye care, the clinics provide services for early identification and treatments for children’s vision problems.

Patients with diabetic retinopathy can receive sight saving care from a retinal specialist, which is available to any CDIB cardholder. For patients suffering from cataracts, an ophthalmologist is available for surgery. Both services are available by referral only. Cataract services are available to Choctaw tribal members who reside within the Choctaw Nation service area.
