Occupational Therapy

Promoting successful participation in daily activities for people with physical, cognitive, social or developmental disabilities or limitations.

Eligibility

  • Must have a medical chart with a CNHSA health facility or:
    • CDIB, letter of descendancy or tribal membership card from a federally recognized tribe
    • Social Security Card
    • Photo Identification Card (Drivers License)
    • Current insurance information such as Medicare, Medicaid or private insurance
  • For children under 19 years of age, the following documents are required:
    • Child's CDIB or a copy of either parent's CDIB if the child has not yet received one
    • State issued birth certificate or hospital birth record issued by an Indian Health Service or tribal facility

How to Apply

Patient must have an occupational therapy consult from a provider within the Choctaw Nation. Therapy ward clerk or occupational therapist will contact patient to schedule appointment once consult is received.

Occupational therapy utilizes daily life activities (occupations) to help people across the lifespan whose ability to function and participate in meaningful occupations is threatened and/or compromised by disease, injury or other mental, physical, social, contextual or emotional impairments. Occupational therapy practitioners promote participation and independence in occupations through the implementation of comprehensive evaluations, therapeutic interventions and discharge planning for improved function in occupations and quality of life.
