Benefit Coordination

Provides one-on-one help with a variety of insurance plans and other federal programs.

Eligibility

  • Social security card
  • Photo identification card (driver's license)
  • For children under 19 years of age, the following documents are required:
    • Child's CDIB or a copy of either parent's CDIB
    • State issued birth certificate or hospital birth record issued by an Indian Health Service or tribal facility

Please contact a benefit coordinator at a local Choctaw Nation Health facility.

Although Choctaw Nation Health Services will continue to provide excellent health care in all cases, please visit with a Benefit Coordinator to learn more about health insurance and the value of being insured. Having insurance coverage provides tribal members with a safety net in the event of a medical emergency, when traveling or away from home, and for any conditions where specialty care might be needed.
