Futures of Football Showcase - Choctaw Nation of Oklahoma

Day of Champions football staff will work together with regional college coaches to instruct and evaluate the players. This is a great opportunity for Choctaw youth (grades 9-12) to be recognized by college coaches in hopes of receiving the opportunity to play at the next level.

The parent workshop will provide information regarding student aid opportunities from federal, state, and post-secondary institutions as well as many other useful resources.

In consideration of me and/or my child or ward being permitted to participate in one or more of the events listed above (“Event”), and on behalf of myself and my child, I hereby acknowledge, understand and agree that:

Participation in the Event carries a significant risk of injury including, but not limited to, the potential for permanent paralysis, other injuries to persons, death, disfigurement or permanent disability, or property damage or loss. These risks may occur due to various circumstances including, but not limited, to the actions of another person or entity, forces of nature, equipment failure, projectile objects, or other causes that may not be known to me.
I am voluntarily participating in the Event, and if my child or ward is participating, I am voluntarily allowing that child or ward to participate in the Event and I, on behalf of myself and of my child and/or my ward, all our heirs, successors and assigns, do assume all risks arising from the Event, whether known or unknown to me.
I agree that both I and my child or ward are of a suitable physical condition to participate in the Event, have sufficiently prepared or trained for participation in the Event, and have not been advised not to participate by a qualified medical professional. I certify that there are no health- related reasons or problems, which preclude either me or my child or ward from participating in the Event. We are aware of our limitations and agree not to exceed them.
If I or my child/ward is in need of medical care and I am unable to consent, I hereby consent to receive emergency medical treatment, as determined appropriate by the Choctaw Nation of Oklahoma. I understand that I will be responsible for payment of all medical expenses to the appropriate provider. I understand and agree that medical or other services rendered is not an admission of liability or an obligation for continued services.
On behalf of myself and my child and/or ward, and all of our heirs, successors and assigns, do agree to FOREVER WAIVE, RELEASE, DISCHARGE, HOLD HARMLESS and AGREE NOT TO SUE OR MAKE ANY CLAIM against the Choctaw Nation of Oklahoma and its elected officials, officers, employees, agents, representatives, volunteers, and contractors (“Releasees”) from any and all liability including, but not limited to, liability arising from (i) any and all injuries to me or my child or ward that may occur incident to any Event and/or any other activity being conducted at the Event location INCLUDING, BUT NOT LIMITED TO, DEATH, DISFIGUREMENT, AND TEMPORARY OR PERMANENT DISABILITY, (ii) any injury, damage or loss arising from or related to the medical treatment authorized in paragraph 4 above, and (ii) any damage or loss of my personal property. THIS RELEASE APPLIES EVEN IF THE INJURY OR LOSS IS CAUSED PARTLY OR ENTIRELY BY THE RELEASEES.
I also agree that the Choctaw Nation of Oklahoma may photograph or capture video of me and/or my child or ward. The Choctaw Nation of Oklahoma may use the images or video footage captured for any purpose, commercial or non-commercial, and in any form desired by Choctaw, in perpetuity and without the obligation to make any payment whatsoever. On behalf of myself and my child or ward and our heirs, successor and assigns, I forever waive all claims based on the Choctaw Nation of Oklahoma’s use of the images, footage, or other likeness. I also agree that the Choctaw Nation of Oklahoma may contact me to obtain follow-up feedback after conclusion of the Event.
I have read this agreement and I understand that, by signing it, I will give up certain legal rights. I guarantee that I am over the age of 18 and have the legal authority and capacity to consent to the terms of this agreement on behalf of myself and the child/ward. I am signing this agreement freely and voluntarily.

Injury/Concussion/General Wavier

In consideration for participation in this program, camp, and/or workout program, I, for myself, my minor child(ren), and our respective heirs, executors, administrators, representatives, agents, and assignees, do hereby forever release and discharge any and all claims, actions, demands, suits and any other rights to recovery, known or unknown for any and all damages, loss or injury that may occur, whether to person or property, as a result of any act or omission, including default or negligence, of the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2025) and all participating sponsors, their members, officers, agents, representatives or employees. This means I intend to release and exonerate all of the above from liability for any damages I sustain or my child sustains. I acknowledge the above-referenced has not forced me to sign this, and I acknowledge there is not a great difference in bargaining power between the parties. The foreseeable possible negligence of the above-referenced could include but is not limited to the following: negligent supervision of the camp attendees, negligence in misconduct, negligence in allowing camp attendees to over-heat, negligence in first aid, negligence in CPR response, negligence the running of drills, negligence maintenance of equipment or other campgrounds, negligence training of camp coaches, negligent retention of camp coaches, failure to recognize the signs and symptoms of heat stroke, failure to recognize the signs and symptoms of concussions, failure to recognize the signs of serious injury, failure to maintain proper medical/first aid equipment, failure to report possible injury and negligence that leads to any foreseeable injury a camp attendee might receive. The foreseeable results of these negligent causes could be anything from minor injuries to serious injury, serious illness, or death. I attest and verify that I have full knowledge of the risk involved with this camp and workout program.

I understand that by signing this waiver, it constitutes a legal signature confirming that having read the above, I attest and verify that I have full knowledge of the risk involved with this camp and workout program for the camper(s) below and as the guardian of listed campers.

Furthermore, I hereby release any and all rights to any photographic, video or audio material DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2025) may wish to release for its event, without obligation whatsoever to me.

I further acknowledge that the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, academy/camp staff, and other academy/camp clients and their families.

I voluntarily seek services provided by the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending the academy.

I attest that:
- That my child/children are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- That my child/children have not traveled internationally within the last 14 days.
- That my child/children have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as noncontagious by state or local public health authorities.
- I do not believe that my child/children have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
- That my child/children or I (Guardian/Parent) have not traveled to a highly impacted area within the United States of America in the last 14 days.
- I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I have not traveled to a highly impacted area within the United States of America in the last 14 days.
- I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
- I have not been diagnosed with Coronavirus/Covid-19 by state or local public health authorities
- I am following all CDC-recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024), or that may otherwise arise in any way in connection with any services received from the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024). I understand that this release discharges the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) from any liability or claim that I, my heirs, or any personal representatives may have against the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) with respect to any bodily injury, illness(COVID-19 infection occurs before, during or after participation in the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024), death, medical treatment, or property damage that may arise from, or in connection to, any services received from the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024). This liability waiver and release extends to the DAY OF CHAMPIONS and the CHOCTAW NATION OF OKLAHOMA (FUTURES OF FOOTBALL SHOWCASE 2024) together with all owners, partners, employees, agents, and representatives.
