On Saturday, April 5, 2025, the myCNHSA app is going away, and is being replaced with the new MyChart app. MyChart makes it easier and more convenient for you to manage your healthcare and your family’s healthcare, giving you a secure, one-stop online access point to health management tools.

Need activation information for a CNHSA MyChart account? The best way to sign up for MyChart is through an activation link you receive when you schedule an appointment or check-in for a service at Choctaw Nation Health facilities.
You can obtain the activation code several different ways:
- Ask a member of our care teams for activation information.
- In some cases, an activation link will be sent or provided automatically. For example, if you have a valid mobile phone number on file, activation information will be sent via test when you schedule an appointment or after you check-in for an appointment.
- If you do not have a valid mobile phone number on file until you activate your account, activation codes may be a part of your printed after-visit summaries.
Please note that in some cases, not all MyChart tools or full functionality is instantly available until after you have completed a scheduled appointment.

myCNHSA App Access
On April 5, 2025, CNHSA switches from myCNHSA to MyChart. If you previously used myCNHSA, it’s important to know that it is no longer available for regular use. You will need a MyChart account to do things like manage your appointments and to message/communicate with your care team.
For a limited time, you will be able to use your active myCNHSA app for read-only access to view certain historical records. This will be turned off and the app will expire on April 5, 2026.

You can use it on your phone, tablet, or computer. In April 2025, we upgraded our electronic health record system to Epic to provide you with even better access to your health information with MyChart.
Yes, MyChart is administered by the electronic health record system, Epic. MyChart allows users to securely view information and use tools like provider messaging.
The switch to MyChart is part of our investment in healthcare that brings the best-in-class electronic health record system, Epic, to Choctaw Nation. MyChart is similar to myCNHSA but offers more tools and a better user experience.
Activating and using MyChart
The best way to sign up for MyChart is through an activation link you receive when you schedule an appointment or check-in for a service at CNHSA. You can obtain the activation information several different ways:
- If you have a valid mobile number on file:
- Look for an activation link to be sent to your mobile phone when you schedule an appointment or check-in for an appointment. Follow the instructions to activate your account.
- If you do not have a mobile phone number on file:
- Look for activation information on your printed after-visit summary received after April 5, 2025. Follow the instructions to activate your account.
- Ask your scheduler, care team or other associate for activation information to be sent via text or email. Follow the instructions to activate your account.
- If you can’t locate the activation information, call 918-567-7034. Representatives can assist with signing up and provide activation links.

You must activate a CNHSA MyChart account to use MyChart. Once your account is activated, you can log in and use its many tools by signing in at mychart.cnhsa.com, using the links included with your activation information or by downloading the MyChart app. For specific usage questions, you can visit the MyChart FAQ page once you are logged in.
Yes, you need an account from each healthcare organization you have a medical chart with.
MyChart tools, Patient Information and Record Access
Unfortunately, we are not able to transfer all information from the myCNHSA app to MyChart. Information such as medication lists, allergies, problem list, etc. are available to providers and may become viewable via MyChart after you complete an appointment, at which time your provider will verify information from our old system. This helps ensure that the information you view in MyChart is up-to-date and accurate, as well as equip providers to provide the highest level of care.
It’s important to know that your medical information and records will never “go away.” Your care team continues to have full access to this information and records. Access to your medical records will always be available through our Medical Records department. For more information on accessing your medical records, call your local Choctaw Nation clinic and ask for Medical Records.
Although you might not be able to message your provider, you can message a member of your care team who will be in communication with your provider to answer your questions or concerns.
MyChart does have online appointment scheduling tools available. But you may not be able to schedule an appointment with all CNHSA providers for several reasons. Certain functions will “turn on” after you have completed a scheduled appointment or used a service in our new system, after April 5, 2025. Also, some providers and specialties are not available for online scheduling via MyChart. Many providers require a referral for appointment scheduling. In that case, please call to schedule. If you have questions about online scheduling, please call your local Choctaw Nation clinic.
