CNHSA clinics provide COVID-19 testing for patients who are presenting any symptoms.

Eligibility

  • CDIB, Letter of Descendancy, or tribal membership card from a federally recognized tribe or:
    • CNO associate

Additional Information

To ensure the most efficient experience during COVID-19 testing, patients should have the following documents with them:

  • Consent Form (if the patient has not been seen at a CNHSA facility)
  • Identification Card
  • Insurance Card (if applicable)
