Family Medicine Residency Clinic
Focused on providing comprehensive medical training to family medicine physicians in order to practice quality, patient-centered and compassionate care for Choctaw Nation patients.
Eligibility
- CDIB, letter of descendancy, or tribal membership card from a federally recognized tribe (living anywhere)
- Non-eligible pregnant women who are pregnant with an eligible child up to 6 weeks postpartum
- Non-eligible spouses of CDIB holders
- Non-eligible health care employees and commission corp
- Non-eligible health care employee spouses and dependent children

Services
- Primary care for all ages
- Yearly physicals and preventative services/screening
- Osteopathic manipulative treatment
- Screening and treatment of hepatitis C
- Contraceptive care including placement of IUDs and hormone implants (Nexplanon), oral contraceptives and barrier protection
- Treatment of migraines with Botox
- Decompression table treatment
- Medication for opioid use disorder (MOUD/MAT)
- Joint injections, paracentesis, skin biopsies and other procedures
Same Day Policy
The CNHSA Residency Clinic can usually accommodate same day appointments. Please call early in the day to ensure availability of openings.
For Prescription Refills
Patients requesting prescription refills should call the outpatient pharmacy at 539-316-6728.
For Scheduled Appointments
Please arrive fifteen minutes prior to your scheduled appointment time in order to be seen in a timely manner. Bring any medications prescribed outside of Choctaw Nation Health Services. If you are unable to keep an appointment, please provide 24 hours’ notice.
