CDC National Diabetes Prevention Program (DPP)

A lifestyle change program to help participants prevent type 2 diabetes.

Eligibility

  • A BMI of 25
  • and one of the following:
    • A history of gestational diabetes
    • An A1C of 5.7-6.4
    • A risk test score of 5 or higher (take the risk test to learn your score)
  • Must be 18 years or older

Additional Information

The Choctaw Nation Diabetes Prevention Program (DPP) is a year-long program. The program is group based with up to 12 people in a group and one lifestyle coach to guide and support participants throughout the program. The lifestyle change program is conducted via WebEx, a virtual meeting platform allowing more flexibility for participants with in person cohorts coming soon in various locations. In this program, participants will learn how to eat healthier in a realistic manner that fits individual needs, how to increase physical activity level and how to manage the stress of everyday life. Participants will set goals, and with the help of a lifestyle coach and group support, reach individual goals and be able to maintain them for a lifetime.

Sessions will be held once a week for 16 weeks, then twice a month for two months, and finally once a month for six months until the year is completed. This program is not intended for those who already have diabetes. If you do have a diagnosis of diabetes and want help making lifestyle changes, contact your local clinic and speak with the Diabetes Educator.

I have been a lifestyle coach for the National Diabetes Prevention Program since 2012 and a Master Trainer since 2018. I started my career with Choctaw Nation in 2001 as Diabetes Educator and became a Certified Diabetes Educator in 2004. I believe small lifestyle changes can delay or prevent diabetes, keeping Choctaw Nation families and community members strong and healthy.

I am a new lifestyle coach for the National Diabetes Prevention Program. I am not new to helping people get healthy! I started my career with the Choctaw Nation in 2002 as a staff nurse. I was blessed to experience many aspects of our Health organization with positions such as a project manager, clinical research coordinator, involvement with the Healthy Marriage/Healthy Kids project, and positions in Behavioral Health. I am excited to involved in this program and ready to help you get and stay health!
